Is Fluoride Toothpaste Bad for You? Separating Fact from Fear

We sell fluoride-free toothpaste, so you might expect us to tell you fluoride is dangerous. We're not going to, because it isn't — and we think you deserve an honest answer, not a marketing one.

The short answer

Fluoride toothpaste is safe and effective for the vast majority of people, and mainstream dentistry isn't divided on this. The American Dental Association requires fluoride in any toothpaste that carries its Seal of Acceptance, and the World Health Organization includes fluoride toothpaste on its Essential Medicines List for treating tooth decay. A 2019 Cochrane review — one of the most rigorous forms of scientific evidence review that exists — confirmed fluoride toothpaste reduces cavities compared to non-fluoride toothpaste. This isn't a minority opinion among dentists. It's the standard of care.

What dental fluorosis actually is

If you've heard fluoride horror stories, dental fluorosis is usually somewhere in them. Here's what it actually is: fluorosis happens from cumulative fluoride intake while adult teeth are still forming under the gums — roughly ages 0 to 8. Once teeth have fully formed, fluorosis isn't a risk anymore, which is why it's a childhood concern, not an adult one.

Even in kids, it's usually mild. National data from the American Academy of Pediatric Dentistry shows about 8% of 12-to-15-year-olds have mild fluorosis — faint white flecking on the teeth, a cosmetic issue only — and severe fluorosis is rare in the U.S. It's also straightforward to prevent: the ADA recommends a rice-grain smear of toothpaste for kids under 3 and a pea-sized amount from ages 3 to 6, supervised, spit-don't-swallow. That's it.

What's real research and what's not

We're not going to pretend there's zero ongoing scientific conversation about fluoride — there is, and pretending otherwise wouldn't be honest either. A 2024 U.S. government report raised questions about a possible link between very high fluoride exposure and children's IQ. It's worth knowing the details: the exposure levels studied were more than double the standard U.S. water-fluoridation level, affecting a small population with naturally elevated groundwater fluoride — and it examined ingested water fluoride, not toothpaste you spit out. The report's methodology was also criticized, including by the ADA's own review process. A large 2026 Wisconsin study following over 10,000 people to age 80 found no association between standard community water fluoridation and adult cognitive function.

So: there's legitimate research on very high, ingested fluoride exposure in specific populations, and that's a real scientific conversation worth taking seriously. What it doesn't show is any credible risk from a normal adult brushing routine with fluoride toothpaste at U.S. water-fluoridation levels. Claims that toothpaste-level fluoride causes thyroid problems or IQ loss in the general population aren't supported by mainstream science.

So why do we make fluoride-free toothpaste?

Because "safe for almost everyone" isn't the same as "the only right choice for everyone." People choose fluoride-free for reasons that have nothing to do with rejecting dental science: taste and texture preference, sensitivity to SLS or other additives common in foaming toothpaste, specific guidance from their own dentist based on their individual cavity risk, or simply wanting a simpler, plant-based ingredient list. If cavity prevention is a top priority for you specifically — because of diet, water source, or cavity history — that's worth discussing directly with your dentist, fluoride-free or not.
